Neurological TOS
Goal of surgical management: To relieve the mechanical load on and subsequent compromise to neurovascular structures in the thoracic outlet
Surgical options:
-Complete or incomplete resection of the first rib
-Anterior and middle scalenectomies
-Excision of fibrous bands compressing nerves or vessels
-Removal of callus formation from previous clavicular fracture

-Complete or incomplete resection of the first rib
- Controversy exists about effectiveness
- Incomplete resection may lead to reoccurrence of symptoms due to inadequate decompression resulting from reattachment of the scalenes, scar tissue development, or bony degeneration off of the remaining tissue remnant
